Prominent cheekbones with hollow temples or lateral cheeks are sometimes described as a peanut-shaped face. The underlying cause may be volume loss, sagging, muscle, fat or bone structure, so adding volume is not always the answer.

WHY WE SEPARATE
Hollow temples and lateral cheeks can accentuate the cheekbones. But when sagging or lower-face volume is also present, adding more volume may make the face feel heavier. This is why the full line is assessed together.

We distinguish a surface shadow from genuine volume deficiency beneath the skin.
Front and oblique views help clarify why the cheekbone appears prominent.
We ask whether tightening should come before adding volume.
Masseter and lower-face volume are considered in the overall contour.
The visual flow from temple to lateral cheek and jawline is reviewed.
SELECT BY ROLE
Filler or collagen-stimulating treatment may support a genuinely hollow area in a measured way.
RF and other tightening approaches may be considered when adding volume is not the first priority.
Botulinum toxin may be considered when muscles such as the masseter meaningfully affect the contour.
Areas with enough tissue volume are not automatically filled; other directions are discussed.
CLEAR DIFFERENCE
When true hollowing is central, we look for a subtle way to reconnect the interrupted contour.
When displaced tissue, muscle or fat dominates, tightening or another option may be more appropriate than adding volume.
We assess front, oblique and side views, then add expression and palpation. Swelling, bruising, tenderness or temporary warmth may occur depending on the selected method, so timing and recovery are planned together.
