Naming the scar is only the beginning. Recording each axis separately makes it easier to see why two scars in the same area may need different priorities.

01 ACTIVITY · A

Activity

We look for recent increases in redness, size, firmness or itching and whether the scar is still changing or beginning to settle. This helps guide timing and treatment strength.

02 CONTOUR · C

Shape and contour

We distinguish raised, depressed, widened or stepped edges and tethering. Height and depth matter, but so does the way the scar meets the surrounding skin.

03 DYSCHROMIA · D

Color change

We separate redness, brown pigmentation, white or lighter areas, and mixed color change. Different causes of color change may call for different methods and timing.

04 SYMPTOMS · S

Symptoms

We ask about the frequency and intensity of pain, itching, tenderness, burning and pulling. Significant symptoms can change priorities even when the visible scar looks subtle.

05 TENSION · T

Tension and friction

We assess forces that can stretch or irritate the scar, including joint movement, pulling along the jaw, chest or shoulder, and pressure from clothing or masks.

06 ANATOMIC RISK · R

Location-related risk

We consider areas prone to keloids, frequently moving sites, skin thickness and nearby structures. A similar-looking scar may need a different approach in a different location.

07 FUNCTION · F

Effect on movement and daily life

We review limited movement, contracture, pain-related difficulty and anatomic distortion. When function is affected, surgical assessment or rehabilitation may take priority over cosmetic treatment.

FROM ASSESSMENT TO PLAN

How a seven-axis record
becomes a treatment plan

  1. Standardize photographs and basic information — We record photographs at a similar distance and lighting, along with when and why the scar formed, its location, previous treatment and symptoms.
  2. Create the seven-axis profile — Activity, contour, color change, symptoms, tension, location-related risk and functional impact are organized by score and type.
  3. Set priorities and treatment candidates — Tension care, vascular or pigment treatment, injections, fractional treatment, adhesion release or surgical assessment are considered in an appropriate order.
  4. Compare the same axes after treatment — Photographs and symptoms are reviewed again to identify what improved, what remains and what to focus on next.

No. The seven axes help identify which features stand out in the scar now. Treatment focuses on the axes that matter for the individual scar.
How does ScarAxis guide treatment choices?
We separate redness and thickness, color change, pain or itching, tension and tethering. This helps identify the first concern to address and the order in which treatments may be considered.
Can ScarAxis be used for an older surgical scar?
Yes. Regardless of when the scar formed, its current color, contour, symptoms, tethering and effect on movement can be reviewed during consultation.
